By completing the form you are requesting one of the following:
- Exemption from one or more of the ongoing eligibility requirements for the Hansen Scholarship while continuing to receive Hansen Scholarship benefits
- Leave of absence from Hansen Scholarship Program (involves not living at Little Hall or receiving benefits)
- Permanent withdrawal from the Hansen Scholarship Program
